Job Description
- Manages and facilitates the complex process of organ and tissue transplantation for patients, serving as a liaison between patients, families, and the medical team to ensure a seamless and supportive care journey from evaluation through post-transplant follow-up
- Patient evaluation, education, coordinating testing, managing documentation, counseling patients and families, collaborating with various healthcare professionals, and providing essential emotional support throughout the entire transplant process.
- Conducting patient and family education on the transplant process and post-transplant care.
- Coordinating comprehensive medical evaluations to determine transplant eligibility
- Providing ongoing support and counseling to patients and families, addressing financial, personal, and logistical concerns
- Acting as the primary point of contact and liaison for patients, families, and the multidisciplinary transplant team.
- Facilitating communication and collaboration among various healthcare providers
- Managing patient caseloads and coordinating care across different departments to ensure continuity
- Managing all relevant medical records and documentation related to transplantation
- Ensuring compliance with legal criteria and ethical guidelines governing organ donation and transplantation.
- For donor coordinators, this involves identifying potential organ donors, managing the organ donation process, and coordinating organ recovery and transport.
- Ensuring the stability of the donor's organs and overseeing the completion of diagnostic tests for organ placement.
